Class Summary




Job Description
The Community Navigator is responsible for being a bridge between health, human, and social service community partners and the residents of Gwinnett County. The Community Navigator provides valuable resources to the residents by gathering information through in-depth conversations. They will help residents navigate issues such as food insecurity, homelessness, and emergency housing, utility and rental assistance and other targeted social needs by giving insight on community organizations best suited to address those needs. The primary role of this Community Navigator is to work with the veteran population and their families.

Essential Duties
  • Coordinate services with relevant community partners, to ensure a holistic approach to meet residential needs
  • Assess resident and community needs using critical thinking and best-case practices
  • Provide appropriate resources based on the needs of those being served
  • Collaborate with key resources within the OneStop facilities and community partners in the surrounding areas
  • Effectively communicate with internal and external stakeholders who work with OneStop 4Help and its strategic program operations
  • Through engagement, provide education and outreach to individuals, groups, and the community raising awareness about the social needs of our residents
  • Maintain accurate and up-to-date records, progress notes, and documentation of services provided
  • Maintain ethical consideration, cultural competency, and professional boundaries to provide excellent customer service and sensitive interactions
  • Remain flexible and communicative in office, field, and other resident interactive settings that inform, help, and sustain resident request and need
  • Excellent written, verbal, and digital communication
Public Speaking Duties
  • Educate and provide information about programs, services, and opportunities to the public at community events, resource and job fairs, and other forums as needed
  • Develop and engage in building relationships through outreach with groups, agencies, organizations, and resident-related interactions
  • Ability to speak knowledgeably and communicate with veterans and other veteran families while working on resource and referral needs

The grade for this position is B241.

Effective 4/11/2023, this position is eligible for the Employee Referral Program.

Minimum Qualifications
  • Bachelor's Degree in social services, social work, sociology, public health, human services, or psychology
  • One year of experience in case management
  • Or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as listed above
  • Valid driver's license
  • CPR certificate
  • First aid certificate

Preferred Qualifications
  • Two years of experience in case management principles, applicable laws, rules, and regulations
  • Active-Duty experience preferred (DD214 required with documentation and Honorable Discharge)

The county's population in 2000 was about 588,448 and we’ve only continued to grow since then, with the population expected to reach one million by the year 2022.

Gwinnett's infrastructure includes more than 2,750 miles of roadways to provide excellent mobility for our residents.

The County Administrator and County Attorney are appointed by the Board of Commissioners and serve at its pleasure. To implement the Board's policy directives, the County Administrator uses a management team consisting of members of his immediate staff and 14 department directors. The 14 departments that make up the executive side are Police Services, Fire and Emergency Services, Corrections, Communications, Support Services, Financial Services, Community Services, Human Resources, Information Technology Services, Law, Planning and Development, Water Resources, Child Advocacy & Juvenile Services, and Transportation. Each department is run by a director who is charged with managing operations in a manner which stresses the County’s values.

In addition to the executive side of County government, certain services are provided through Constitutional officers and independent elected officials. These external offices are created by the Georgia Constitution or through state law. In Gwinnett, they include the Sheriff, Tax Commissioner, District Attorney, Solicitor, Probate Court Judge, and Chief Magistrate.
