Job Posting for Case Manager at H & A Counseling Associates, PSC

H & A Counseling Associates is seeking a motivated Targeted Case Manager to work with children, adolescents, adults and families in Harlan County, Kentucky. We are a private practice that values our professionals and treats them with respect and support. The Targeted Case Manager will assist the client in gaining access to needed medical, social, educational and other services and work in tandem with therapists to provide comprehensive and well-rounded care.

Duties

  • Complete written needs assessments in collaboration with the client.
  • Assist therapists with the development of treatment plans
  • Link client to needed services in accordance with their needs assessment and treatment plan and assist with eliminating any barriers to those services
  • Become familiar with community resources
  • Make all required monthly contacts with client
  • Complete proper documentation of services rendered

Education and Requirements Bachelor's Degree in a Human Services or Psychology related field is required

  • Experience in working with area population is strongly preferred
  • Previous Targeted Case Management experience is preferred
  • Computer skills are required
