POSITION GOAL
The Health Alliance Nurse will provide health education, social assessments, breastfeeding support and follow-up to pregnant women and post-partum women, including depression screen. This will be done in the clinic setting, as well as through home visits. In addition, the Health Alliance Nurse will provide all aspects of the nutrition education component and process of assessing for nutritional risks as associated with the WIC and Maternal Health Program. SCHEDULE AND WAGE
Monday-Friday 8:00 am - 4:30 pm. Eventually you will be required to work until 6:30 pm twice per month.
Wage is $27.38 - $30.90 hourly depending on education and experience.
- This position is eligible for a $1200 retention incentive after successfully completing 180 days of employment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provide Nursing services for the Health Alliance within the clinic or home setting.
- Provide Health Education for pregnant and post-partum women.
- Complete a social assessment of families and pregnant/postpartum women.
- Provide assessment, education, documentation, dental screening/fluoride varnish, referral, and follow-up services to Health Alliance clients.
- Support family efforts to follow through with established goals on the maternal health form, school/employment efforts, and establishing positive social outlets.
- Perform direct activities of family-centered care coordination to include: (1) identifying resources the family may need, (2) informing families about resources, (3) assist in an application for health insurance (4) bridging communication gaps, (5) serving as an advocate for the family, (6) arranging transportation, if needed, (7) providing support and assistance as families negotiate the system, and (8) enabling families to become knowledgeable, independent health care consumers.
- Provide accurate documentation follow-up of referrals made.
- Conduct and review the results of the following tasks necessary for assessing nutritional risk: a) questionnaire; b) measurement of length/height and weight; c) hemoglobin/lead; d) health questionnaire.
- Provide in-depth individual nutrition counseling as needed or requested by a member agency, a participating family, and/or staff working with the family.
- Serve as a staff person qualified to provide nutrition information to interested professionals, outside agencies, and individuals.
- Provide nutrition services to the Maternal and Child Health programs as assigned.
- Assist in tailoring the WIC program food package to best meet individual participants' dietary needs.
- Attend and participate in state nutrition meetings and other conferences that serve to enhance training and skills.
- Assist WIC coordinator in the development of needed program evaluation tools and plans.
- Function as a member of the agency chart audit team annually.
- Provide breastfeeding education, support, and guidance to clients.
- Assist the WIC Coordinator in the preparation of the WIC portion of the annual grant application.
- Assist with missed appointment reports.
- Responsible for special formula documentation and ordering of specialized formula.
- Follow procedures, action plans, and guidance to assure compliance with the Iowa Department of Health and Human Services (HHS) Regulations.
- Maintain current data in state data systems.
- Follow HACAP procedure for reporting suspected child abuse and neglect.
- If needed, act as a community case coordinator for Head Start/Early Head Start families.
- Manage a flexible work schedule according to the agreed-upon number of hours scheduled to work per week, in conjunction with the supervisor that meets the needs of the company, clients, contractors, and any other appropriate parties. This may include occasional evening and weekend hours.
QUALIFICATIONS

Education and/or Experience
- Bachelor degree in a health or human services field
- Current license as a registered nurse (RN) with a bachelor s degree in any field
- Current license as an advanced registered nurse practitioner (ARNP)
CONDITIONS OF EMPLOYMENT
- Complete Child Abuse Reporter training within six (6) months of employment and update every three (3) years.
- Complete bloodborne pathogen training within ten (10) days of employment and update every year.
- Valid Iowa driver s license.
- Valid car insurance.
- Reliable transportation for on-the-job use.
