The HIV Counseling and Testing Coordinator oversees the Counseling & Testing and Community Health Workers Program within the A-Team. The Coordinator schedules testing and education events, and Community Health Worker Activities throughout the community. The Coordinator ensures there is adequate staffing for testing events and community outreach and that all information is reported to the appropriate entities in a timely manner. He/She is also responsible for providing testing and counseling to clients.
Schedule: Monday, Tuesday, Wednesday 8 am - 4:30 pm / Thursday 12:30 pm - 9 am / Friday 8 am - 4:30 pm / Every other Saturday 8 am - 4:30 pm with one day off during the week.
Responsibilities
A day in the life of an Aids HIV Ambulance Care Coordinator at Hackensack Meridian Health includes:
- Coordinates Community Outreach Services: As assigned, provides services at high schools/ colleges/ churches/ clubs and other organizations.
- Provides services to clinics within the Family Health Center.
- Maintains records in an organized manner.
- Follow NJSDOHSS regulations.
- Consistently demonstrates an ability to learn new skills and routines, and to perform them properly.
- Explains collection procedures and provides general testing within a timely manner.
- Responds to all assigned appointments and walk-in patients.
- Travels off-site to assigned locations.
- Represents HMH at scheduled events and actively promotes hospital and partner company services.
- Ensures a good Show at sponsored events.
- Acts as a liaison for community organizations in planning and implementing community health projects.
- Understands and follows all NJDOH policies.
- Ensures all information is reported accurately in a timely manner to the appropriate entities.
- Fills out all required forms accurately and in their entirety as required by the NJDOH.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- High School Diploma or equivalent
- Must be proficient in computer data entry.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
- Prior experience as an outreach worker.
- Prior experience working with HIV patients.
