SUMMARY
The Patient Care Coordinator, LPN is responsible for providing and assisting with patient care under the direction of a physician or licensed health care provider in a clinic setting.
PRIMARY RESPONSIBILITIES
1. Responsible for care management between patients, physicians and outside agencies. (referrals, pharmacies, etc.)
2. Lab culture reviews and Infection Control.
3. Reports results and pertinent information to patients and physicians.
4. Evaluates understanding of treatment plan and/or instructions.
5. Assists physician as needed.
6. Documents basic assessment and care/instruction given as needed.
7. Obtains precerts/prior authorizations for tests, procedures, medications, and DME if required.
8. Complete and process Home Health and Hospice forms.
9. Complete FMLA and disability forms.
10. Assists with a variety of procedures, exams and/or operation of diagnostic equipment as assigned and room patients if needed.
ADDITIONAL RESPONSIBILITIES
1. Maintains pharmacy inventory including drug samples.
2. Calls in prescriptions to pharmacy.
3. Provides direct patient care as needed.
4. Attends mandatory meetings.
5. Adheres to the dress code, attendance and all other company policies and procedures.
KNOWLEDGE AND SKILL REQUIREMENTS
1. Must be a graduate of an accredited school of nursing.
2. Current LPN licensure for the State of Alabama.
3. Current CPR certification.
4. Proficient oral and written communication skills.
5. One year experience as a LPN in a clinic setting preferred.
WORKING CONDITIONS
1. May be exposed to the risk of communicable diseases and radiation.
2. May require handling of unclean or infectious materials.
3. Normally works in a well-lighted, well-ventilated area.
4. Must be able to work under the stress and conditions that normally exist in a clinic setting.
5. Classification Category 1 - Tasks that involve exposure to blood, bodily fluids, or tissues.
