Description
As a member of the Medical team, this position is responsible for providing primary medical and mental health care to patients of HCHN. This position will provide direct care for 95% of the time, and administrative responsibilities 5% of the time.
Providers must understand and support the vision and mission of HCHN. Understands that employment is contingent upon adherence to HCHN policies and procedures, JCAHO, OSHA and HIPAA rules, and regulations - related codes and laws.
General responsibilities
- Performs medical and mental health examinations, diagnoses illnesses and treats patients in accordance with their medical needs.
- Advised patients on good health practices.
- Ensures patients receive educational materials that pertain to their patients health needs.
- Ensures all patient charts are completed within 24 hours of the patient visit, and the charting is comprehensive and legible.
- Refers patients to specialists when required and in accordance with their insurance restrictions.
- Ensures patients in need of behavioral health services are routed to the behavioral health team.
- Ensures all patient charts are completed within 24 hours of the patient visit, and the charting is comprehensive and legible.
- Effectively and efficiently completes all paperwork requirements for billing and medical records compliance.
- Performs procedures for patients in need. Develops and implements goals and objectives to ensure high quality, cost-effective clinical care.
- Plans and implements policies, procedures, and systems to achieve stated goals and to ensure compliance with JCAHO and other regulatory/third party mandates.
- Regularly advises the management team on issues regarding operations and patient care to include staffing, data collection, and interpretation clinic flow, appointment systems, client recall process, ancillary services, and customer satisfaction.
- Participates in the effectiveness review of the clinical operation activities which include but are not limited to, patient flow, laboratory services, x-ray services, pharmacy services, referrals, and all other activities within the clinics.
- Implement procedures and clinical programming that would improve the continuum of patient care in an ambulatory setting.
- Participates in management and clinical reviews directed by BHCDA, PHS Region V., State of Michigan FIA, Michigan Department of Community Health and any other applicable Federal, State or Local regulatory agencies.
- Responsible for all CLIA requests, formulary coordination, and other clinical practice protocols.
- In accordance with grant requirements, adheres to the HCHN Health Care Plan as evidenced by compliance with the standards within the plan.
- Participates in community and organizational activities designed to modify community health-seeking behavior, epidemiology, and community health care needs.
- An active participant in HCHNs Performance Improvement Process, Providers meeting, and other committees as scheduled.

Qualifications
Education and/or Experience
- Licensed Nurse Practitioner to practice medicine in the State of Michigan.
- Psychiatric/Mental Health NP certification
- Comprehensive knowledge of all phases of health care delivery, as well as the theory and practice of organizational management, preferably in health care and/or not-for-profit environment
