What are the responsibilities and job description for the Case Manager position at Harbor Brand?
Harbor is seeking a Case Manager who will be responsible for the activities of clients with substance abuse and behavior health problems. Responsible for assisting clients in obtaining the necessary living/recovery skills to achieve their highest potential.
Position is full-time
Education/Experience/Other Requirements:
- Minimum of a High School Diploma and 3 years of experience in a behavioral health or related field required. Associate’s Degree in Counseling/Social Work/ Related Field or Bachelor’s degree in human service field or equivalent experience preferred
- Chemical Dependency licensure required within 90 days of employment
- Knowledge of 12-step groups of recovery.
- Must have valid driver’s license, acceptable driving record, and be deemed insurable through Harbor’s insurance carrier.
o For residential programs; Must be at least 23 years of age, have reliable transportation, must remain up to date on annual Defensive Driving for 15-Passenger Vans training requirement. - CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
- Must be proficient and accurate in computer use, including Microsoft Outlook and Word.
Essential Job Competencies/Primary Duties:
- Schedule and meet with each client as needed, develop treatment plan, and schedule aftercare appointments.
- Organizes, schedules, and coordinates all case management activities including transportation to appointments.
- Conducts groups and individual services to provide education about recovery and to support clients implement their Relapse Prevention Plan.
- Completes all appropriate paperwork for client charts.
- Understands and maintains professional boundaries in client interactions.
- Participates in interdisciplinary treatment planning.
- Networks with outside agencies, as needed, to ensure individual client needs and goals are met.
- Presents workshops to other agencies on women’s specific recovery issues, as needed.
- Participates in training and development programs to enhance knowledge, skills, and personal service network.
