The Case Manager is responsible to affect a safe, comfortable and supportive environment for the patients participating in the program. Helping clients with legal, financial and life skills. Teaching groups to the clients on life skills, coping skills, how to maintaine sobriety, ect.
Bachelor’s Degree Required in any of the following:
Psychology
Social Work
Criminal Justice
Political Science
Sociology
Human Services
Board of Regents with Health Specialization or Recreational Therapy
At least six (6) months experience working in the Human Services/ Recovery field, preferred.
Must be available to work some weekends as well as throughout the week.
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
- Monitors each patient throughout the daily schedule.
- Enforces program rules and expectations and reports infractions to the Therapist and Clinical Director.
- Monitors patient conformance with program schedule, reports problems, and informs supervisor.
- Makes daily assignment checks and discusses results with each patient.
- Monitors patient participation and reports problems to the supervisor.
- Resolves conflicts through general and supportive advice.
- Participates in patient recreation, leisure, and weekend activities as scheduled.
- Provides case management services.
- Transports patients to scheduled activities or arranges transportation.
- Reports any vehicle problems to supervisor.
- Prepares educational and motivational lectures and groups.
- Monitors AA/NA meetings on-premises including recruitment of speakers.
- Maintains the confidentiality of all patient and facility activities.
- Complies with Policies and Procedures.
- Accepts and utilizes supervision
- Addresses problems noted by supervisor.
- Performs other assignments as requested by supervisor.
- Demonstrates willingness to accept responsibility
- Completes all tasks with concern for work quality
- Being sensitive to the Cultural Issues of a patient based on their job position
- Reporting Suicidal/Homicide of a patient based on their job position.
- The ability of competency and understanding to Addition based on their job position.
- Reporting and responding to issues of Trauma of a patient based on their job position.
- Reporting and responding to issues of Dual – Diagnosis issues of a patient based on their job position.
Job Type: Full-time
Pay: $16.50 - $18.00 per hour
