Responsibilities
The Patient Care Coordinator (PCC) focuses entirely on aiding the Provider in taking care of patients. As a Patient Care Coordinator you will:
- Serve as the point person for the provider’s patients, facilities, and other healthcare professionals
- Manage incoming calls and collect all pertinent medical information
- Responsible for processing provider orders
- Schedule consult and specialist appointments
- Request medical records; review and assign faxes and attachments
- Attend regular meetings and in-services, participates in committee meetings and professional development activities
Qualifications
- Must be a Licensed Practical Nurse/Licensed Vocational Nurse
- Must maintain a valid driver’s license and good driving record
- Active CPR Certificate
- Minimum 2 years of office experience in health care coordination
- Basic working knowledge of medical terminology
- Microsoft Office skills
- Strong customer service, organizational, and interpersonal skills, with excellent multi-tasking and prioritization
- Ability to communicate effectively with all personnel levels within the organization
