Position Overview:
The Healthy Start Registered Nurse (RN) Care Coordinator will support the implementation and management of the Healthy Start initiative within Harris County Public Health (HCPH). This role involves coordinating patient care, supporting health education efforts, and ensuring program objectives are met to improve maternal and child health outcomes. The RN will provide safe, compassionate nursing care as a member of the care team, providing patient-centered care, patient safety advocate and member of the health care team. The RN is required to function within the parameters of the legal scope of practice of an RN, and in accordance with the federal, state and local laws; rules and regulations; and policies, procedures and guidelines of HCPH. Services may be provided in participant home, clinic, or through telehealth modalities.

Duties and Responsibilities:
- Coordinate and manage patient care activities, including scheduling appointments, follow-ups, and ensuring continuity of care for program participants, delivered in participant homes, clinics, or via telehealth.
- Conduct comprehensive health assessments for program participants, including maternal and child health evaluations and screenings, in various settings.
- Develop and implement individualized care plans for program participants, addressing specific health needs and goals.
- Provide health education and counseling to program participants on topics such as prenatal care, nutrition, and child development, in both individual and group settings, using in-person and telehealth modalities.
- Collect and maintain accurate patient records and data, ensuring compliance with confidentiality and reporting requirements.
- Support community outreach activities to promote Healthy Start services and recruit program participants.
- Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate care and services for program participants.
- Identify areas for improvement in patient care and support the implementation of quality improvement initiatives.

Education:
- Bachelor's degree in Nursing (BSN)
- Current Registered Nurse license in the State of Texas by the Texas Board of Registered Nurse Examiners with no restrictions
- American Heart Association CPR Certificate
Experience:
- One (1) year of experience as an RN providing women's health services
Licenses:
- Valid TX driver's license
Knowledge, Skills and Abilities:
- Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook) and navigating Electronic Medical Record Systems.
- Excellent communication skills (verbal & written)
- Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
- Ability to organize material, prioritize job tasks, make reliable judgments, and draw conclusions from verbal discussions and written data
- Ability to draw conclusions from data, such as lab tests, to record this information accurately in patient’s record, and to notify medical providers when deviations occur
