Job Summary:
You would be joining the Ayer Neuroscience Institute at Hartford Hospital in the position of Stroke Coordinator. This role has the benefits of being part of the Institute along with the resources and teamwork that it brings. Your role, in partnership with the interdisciplinary team, would be to support, manage and coordinate systems related to acute stroke care delivery at Hartford Hospital, optimizing the care and access.
In conjunction with the Manager of the Stroke Program at Hartford Hospital, the Stroke Coordinator is responsible for the development, implementation, coordination and evaluation of the stroke program as well as the management, education and current evidence-based practice related to stroke care and prevention. In addition, the coordinator adheres to the clinical practice guidelines set forth by the Medical Director and the Joint Commission standards for quality care.
The coordinator also provides support for the stroke program to medical and nursing staff and those departments, which have direct and indirect patient care responsibilities. The Stroke Coordinator supports patient, assisting them in the navigation through the care process. Lastly, the stroke coordinator is responsible for problem identification, problem solving, education, research, and performance improvement including QA/QI initiatives for the program.
