LVN Care Manager

Haven Health, a well-established home health agency, is searching for an LVN interested in an office position in which they are facilitating patient services! You and your team will coordinate, collaborate and manage patient care from the office, according to the care plan and work towards better patient outcomes.

All new employees are given the option of determining when they are ready for independent performance. We will provide all the training needed and facilitate proficiency in tasks for clinical staff.

Position Available:

Full Time

Responsibilities:

· Coordinate scheduling and access to patients for those working in the field

· Facilitate access to supplies and equipment needed by patients

· Place orders and request authorization for needed services

· Collaborate with RNs, LVN’s and MSW’s to formulate and carry out the care plan

· Field visits 1-2 x weekly as directed by RN supervisor.

Qualifications:

· Current active California LVN license

· Excellent written and verbal communication skills

· Licensed driver

· Successful completion of a background check, drug test, and physical

· Bi-Lingual Spanish preferred
