Job Description: The LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care. Duties and Responsibilities: Develops and maintains on-going relationships with management, department staff, case workers, patients, and family members in order to best meet the needs of clients. Ensures that delivery of nursing services is compliant with company/state policies. Available resource person to other health team members in the identification and fulfillment of patient needs. Maintains liaison with the community’s health-related resources. Qualifications, Knowledge, Skills, and Abilities: Graduate of approved School of Professional Nursing - or - Current state Licensed Practical Nurse license. Pediatric nursing experience preferred.
