SHIFT: Days (rotating weekends)
SCHEDULE: Full-time


Case Management Assistant
 
POSITION SUMMARY:

The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.

 

ESSENTIAL FUNCTIONS (including, but not limited to):

  • Distribute the Important Message letters to the patients/families and explain the reason for receiving it.
  • Distribute the Beneficiary letters to the patients who need it and explain why they are receiving it.
  • Must be able to send out referrals to the Post Acute Care providers via Midas.
  • Obtain a Choice letter with the preference of Post Acute Care from the patients.
  • Assist in getting the 3008 forms signed by physicians for the Skilled Nursing Facilities.
  • Call the insurance companies to follow up on Post Acute Care auths.
  • Other duties assigned as required. 

 

QUALIFICATIONS: 

  • Associates Degree preferred
  • Medical knowledge and terminology required
  • Minimum of one year experience in a health care setting required
  • Health care professional (such as CNA or EMT) preferred
