SHIFT: Days (rotating weekends)SCHEDULE: Full-time
The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
ESSENTIAL FUNCTIONS (including, but not limited to):
- Distribute the Important Message letters to the patients/families and explain the reason for receiving it.
- Distribute the Beneficiary letters to the patients who need it and explain why they are receiving it.
- Must be able to send out referrals to the Post Acute Care providers via Midas.
- Obtain a Choice letter with the preference of Post Acute Care from the patients.
- Assist in getting the 3008 forms signed by physicians for the Skilled Nursing Facilities.
- Call the insurance companies to follow up on Post Acute Care auths.
- Other duties assigned as required.
QUALIFICATIONS:
- Associates Degree preferred
- Medical knowledge and terminology required
- Minimum of one year experience in a health care setting required
- Health care professional (such as CNA or EMT) preferred
