Social Worker II - Behavioral Health
SCHEDULE: Full-time, Days (rotating weekends)
POSITION SUMMARY:
Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planning. Works in collaboration with the Case Manager to identifies potential barriers to patient throughput , and quality outcomes. Works within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.
QUALIFICATIONS:
- Master’s degree in Social Work or Counseling Psychology required
- One of the following Florida licenses is required:
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- One year experience in discharge planning in a health care setting required
- Basic Life Support (BLS) required
SKILLS, KNOWLEDGE, AND ABILITIES:
Proven ability to work with others, at all levels within the organization and collaborate effectively. Possess interpersonal problem solving skills and written/oral communication skills. Computer literacy. Ability to complete assessments within the behavioral health field such as but not limited to: psychosocial assessments, discharge assessments and intake assessments.
