JOB SUMMARY

GENERAL POSITION DESCRIPTION: 

The Philadelphia Department of Public Health (PDPH) Division of Chronic Disease and Injury Prevention (CDIP) works to make safe and healthy living easier for residents. Our work identifies and addresses health inequities and is guided by a commitment to health justice and the disruption of historical patterns that continue to contribute to structural racism, resulting in disparities in health outcomes and well-being. We recognize that health is shaped by structural factors including systemic racism and other social, economic and political forces; thus, we work to change systems, policies and environments to improve health, rather than focusing exclusively on health promoting behaviors. We take a data-driven approach to program and policy design, looking to evidence-based solutions when possible and contributing to building that evidence base with our own research. We work collaboratively with partners across the City, including Philadelphians directly affected by any issue. Chronic disease focus areas include preventing disease related to tobacco, poor nutrition, lack of physical activity, inadequate housing, poor working conditions, low incomes, and other known risk factors. Our injury prevention work focuses on the gun violence crisis in Philadelphia.

We are seeking an individual with an intimate knowledge of Philadelphia, who has experience cultivating robust and meaningful community partnerships for the Health Equity Coordinator position. This position will report to and work closely with the Health Partnerships Program Manager and will collaborate with the Health Partnerships Project Coordinator to support CDIP’s work on the Philadelphia Innovative Cardiovascular Health Program (Cardiovascular Health) funded by the CDC to improve health outcomes of Philadelphians with hypertension, high cholesterol, and cardiovascular disease while addressing social determinants of health and reducing health inequities. The Health Equity Coordinator will be dedicated to reducing care disparities and advancing health equity, particularly in identified communities of focus in Philadelphia. This role will involve fostering partnerships and coordination among grant partners participating in a Heart Disease and Stroke Learning Collaborative, identifying the greatest social service and support needs in the communities of focus, and leveraging the Learning Collaborative network to help address identified barriers and needs. The Health Equity Coordinator will also support CDIP’s health justice work and serve as our liaison to PDPH’s Chief Racial Equity Officer and team (CREO) for strategizing, sharing best practices, and coordinating with PDPH’s other health equity efforts.

This position is contingent on continued grant funding.

JOB SPECIFICATIONS

Responsibilities/Duties

  • Meet, communicate, and actively engage with grant partners regularly to get updates, provide technical assistance, and track progression of grant activities.
  • Plan, coordinate, and conduct learning collaborative meetings in partnership with collaborating partners.
  • Outreach to and recruit organizations within the community to serve as partners for the Cardiovascular Health Program to address social needs within the communities of focus where hypertension prevalence is high among adults.
  • Serve as the touchpoint with partners for mapping social needs within the community
  • Assist with facilitating partnerships between community organizations and health systems.
  • Provide technical assistance to partners that utilize referrals to connect patients and community members to resources to address social needs.  
  • Research culturally informed, evidence-based interventions and policies around cardiovascular health and disseminate them to partners.
  • Attend and actively participate in meetings within the division and with partners across various sectors including health care, community organizations, payors, and government partners. Assist with meeting coordination, preparation of meeting agendas, and meeting minutes.
  • Work with Health Partnerships team and partner organizations to assess progress on hypertension and cholesterol control and process measures related to achieving target metrics.
  • Support evaluation activities, which could include conducting interviews, for example, to assess impact and effectiveness of the program.
  • Coordinate efforts with the CREO office and participate in department-wide health equity workgroups to ensure implementation of best practices in healthy equity approach.
  • Participate in cross-divisional trainings to grow skillset and knowledge in the areas of equity and community engagement.
  • Adopt and support CDIP with adopting and integrating a health justice and equity approach for all our work.
  • Assist with any additional activities required to support the goals of the grant and CDIP.

Qualifications


Education

  • Completion of a bachelor’s degree required, master’s or strong work experience preferred, with educational background in public health, nursing, health education, community health, health equity, social work, or another related health profession from an accredited college or university.
  • Two or more years of work experience with community health practice, community engagement, community organizing and/or advocacy.
  • Demonstrated understanding of structural racism and how it influences social determinants of health which impacts health outcomes and disparities.
  • Demonstrated persuasive writing skills and ability to organize written communication for a variety of audiences.
  • Demonstrated ability to present ideas clearly and confidently in small and large group settings.

OR

Any equivalent combination of education and experience determined to be acceptable by the Department of Public Health.

Skills/Experience

  • Must live in Philadelphia or be willing to relocate to Philadelphia within 6 months of hire.
  • Ability to develop and foster partnerships by engaging in community-based outreach.
  • Experience working in the social impact space or complex service environments.
  • Excellent oral and written communication skills with different audiences.
  • Ability to manage projects independently, thoughtfully, and to completion.
  • Ability to manage multiple tasks and deadlines.

Work Environment:

The Health Equity Coordinator will work primarily in a standard office setting with extended periods at work station and periodic use of office equipment. The role will also involve time in the community engaging with community partners. A combination of in-office and remote work may be allowed per City and Department policy.

Position Type and Work Schedule:  Full-time position, typical hours are Monday through Friday 8:30 am to 5:00 pm.

Travel:  Some local travel between sites to attend meetings and meet partners.

Physical Demands:  Limited physical demands.

Salary and Benefits:

  • $58,000 - $63,000
