Health1st Home Health Services (HHHS) is seeking Skilled Home Health Nurses, both Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join their growing team. Skilled home health nurses care for clients in the comfort of their own homes. A variety of shifts are available in the  Delaware County and surrounding Suburbs . HHHS nurses choose their work hours!. Nurses select what days of the week they are available for work. QUALIFICATIONS:

1. Graduate of a state-approved school of practical nursing & currently licensed in the state of Pennsylvania
2. Minimum of one (1) year experience in nursing, preferred.
3. Acceptance of philosophy and goals of this HHHS.
4. Ability to exercise initiative and independent judgment.
5. Compassionate, kind, and gracious spirit

RESPONSIBILITIES:

1. Understands and adheres to established policies and procedures.
2. Implements the nursing care plan for each Client.
3. Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
4. Initiates preventive and rehabilitative nursing procedures as appropriate.
5. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the Client's physical or emotional condition.
6. Teaches and counsels the client and family/significant others regarding the nursing care needs and other related problems of the Client at home
7. Maintains accurate and complete records of observations, treatments, and care of the Client.
8. Participates in medical record audit as assigned.
9. Attends staff meetings, Client care conferences, and in-services as scheduled.
10. Participating in Client care conferences to discuss the need for involvement of other members of the health team, such as a physical therapist or speech-language pathologist.
11. Prepares clinical and progress notes
12. Prepares equipment and materials for treatments.
13. Observes aseptic technique as required.
14. Assists the Client in learning appropriate self-care techniques.

WORKING ENVIRONMENT:

Works indoors in HHHS office and Client homes and travels to/from Client homes and may travel with client when necessary.

LIFTING REQUIREMENTS:

• Ability to participate in physical activity.
