Our organization is seeking MDS Coordinator to help create an exceptional experience for resident and family through care coordination. Our MDS Coordinator will ensure that clients are provided with the best possible outcomes and our company is able to be a good steward of the community.
Responsibilities:
- Plan, direct, or coordinate the activities of the medical department, including administrative and clerical work, patient intake and discharges, transfer of records to other facilities, and patient education.
- Monitor patients' health condition and physical status throughout their stay at the facility.
- Perform admission screening exams and routine health assessments.
- Maintain records, order supplies, keep financial files up to date, and prepare statistical reports on hospital operations.
- Perform medical recordkeeping activities.
- Prepare patients for procedures or surgery.
- Create an environment that fosters ongoing communication between patients and staff.
- Prepare patients for discharge from facilities by coordinating with other departments., Create a care plan and help coordinate hospice care.

Pay: $85,000.00 - $90,000.00 per year
