Field RN Case Manager

HealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune 500 Healthcare Organization in Toms River, NJ.

Daily Responsibilities for Field RN Case Manager:
  • Ensure accurate and complete documentation, assist with benefit determination
  • Travel to 3 locations (typically 1 Location per day)
  • Monday – Friday 8am – 5pm
     
Required Qualifications for Field RN Case Manager:
  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in New Jersey
  • Certified in Basic Life Support
  • 1 year working in a Long-Term Care facility/ Med-surg/ ER experience as an RN
  • COVID-19 Vaccine

Preferred Qualifications for Field RN Case Manager:
  • 1 year of clinical experience in practice (long-term care setting preferred)
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness

Pay Details: $47 - $49 an hour
