Daily Responsibilities for Bilingual Spanish Long Term Care Manager - Hybrid:
- Responsible for members gaining access to needed services through coordination and integration of medical and long-term care services for orientation, care plan development, assessment, and care coordination.
- They will be assessing needs of elderly patients-i.e. asking them questions to see what type of services they need for expr home health care
- Working with the impoverished elderly population (55 yrs. old).
- M-F 8AM-5PM. Up to 80% field based
- Must be Bilingual Spanish
- HS Diploma and 6 years of case management experience or Bachelor’s degree and 2 years of case management experience
- OR LPN with 4 years of case management experience
- Must be comfortable going into members’ homes for assessments and able to handle caseloads of 40 members
- Must have experience with EMR
- Must have a Valid and Active Driver’s license
- Must have MS Office exp - excel, outlook, word
- Knowledge of healthcare and managed care preferred
- Home health, long term care exp or Discharge planning exp
- Exp working for the Department of Children and Families
- Immediate enrollment in Health Insurance
