HealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Managed Care Plan in Waco, TX.
Daily Responsibilities for Field RN Case Manager:
- The Field RN Case Manager will in the field 4 days per week and 1 remote day
- Potential for face-to-face assessments in member home within the Managed Care Services AREA(MRSA)
- The Field RN Case Manager will be complete all documentation in the electronic health record; referrals for items/services, coordination of care
- The Field RN Case Manager will befollowing up on members in the upgrade process to implement services
- The Field RN Case Manager will be making outbound phone calls to members and providers to coordinate services
- This will be a Contract role for 1 year, schedule is Monday to Friday 8AM -5PM.
- Active State RN License in the State of TX
- Must be residing in College Station or Waco, TX
- Case management experience
- 2 years of clinical nursing experience primarily with adult/geriatric population
- Ability to use Outlook, Skype, Excel, and Zoom needed.
- Reliable transportation, clear driving record, and insurance
Preferred Qualifications for Field RN Case Manager:
- Previous field or home health experience
- Bilingual in Spanish
- Current RUG certification
