- Must possess levels of knowledge of psychosocial therapies and social casework generally acquired through completion of a Master's Degree from an Accredited School of Social Work (MSW).
- Must be licensed, or eligible for licensure, by the State of Michigan
- Requires a high level of analytical abilities in order to assess patient needs, develop associated treatment modalities, and provide effective counseling in resolving complex cases encountered with patient caseload.
- Work requires a high level of interpersonal skills to provide counselling to families and patients in time of great stress and to relate will with other facility personnel who also work under stress.
Performs the full range of social work assignments in a long term care setting including but not limited to:
- Conduct an admission interview with residents and/or families to determine resident’s social, psychological, cultural and family situation in order to assess factors which may interfere with or contribute to resident obtaining maximum health care benefits.
- Seek out residents who may be candidates for discharge to their homes or to a lesser level of care and provide discharge planning services including contact with outside services and facilities, providing information needed.
- Update resident’s discharge plan quarterly and resident's psychosocial histories and next of kin information annually.
- Provide the inter-disciplinary health team with psychosocial information about resident and recommends plan of action for meeting resident's psychosocial needs at care planning conference to facilitate coordinated treatment of resident.
- Interpret social and family history information for attending physician and other resident care team members.
- Provide counseling to resident and/or their families and encourage family involvement.
