What You'll Do
Currently, we are looking for Registered Nurses to join our team who would like to combine their love for people and excellent clinical skills with the opportunity to advance your career.
Our Registered Nurses plan, organize, and provide exemplary nursing care to our patients. They build from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.
To be successful in this role you will:
- Complete initial, comprehensive, and ongoing comprehensive assessments of patients and families to determine patient needs.
- Develop care plans that establish goals, based on nursing diagnosis, and incorporating palliative nursing actions. Including the patient and the family in the planning process.
- Communicate with community health related persons to coordinate care plans.
- Work in cooperation with families/caregivers and interdisciplinary groups to meet the emotional needs of the patient and family/caregiver.
Who You Are
** Must have current Registered Nurse and CPR certification; One (1) year of experience working under your RN license; Minimum 1 year experience working in home health; A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
