Job Description    

The RN is responsible to the Nurse Manager(s) and is responsible for coordinating care of residents using the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with residents, family, and other staff while maintaining standards of professional nursing.

Reports To: Nursing Manager(s)

Coordinates care of residents using the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with residents, family, and other staff while maintaining standards of professional nursing.  Supervises staff as assigned.

Position Responsibilities/Standards

  • Utilize the nursing process by demonstrating the appropriate assessment, planning, implementation and evaluation of the resident’s care.
  • Assess the resident; collect and document the resident’s current health status and medical history.  Make accurate and ongoing assessment of resident’s status and respond appropriately.
  • Plan/Implement; set priorities for nursing action in a logical sequence according to resident’s needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards.  The Plan of care is initiated on resident admission based on all aspects of the initial assessment.  Plan includes physiological, psychosocial, and environmental factors: resident/family education and discharge planning.
  • Evaluate and perform ongoing assessment and revise plan of care based on new data and resident’s condition.
  • Document accurate and ongoing assessment of resident status.  Document patient care which reflects nursing intervention, resident response to care provided, resident needs, problems, capabilities, limitations, and progress towards goals.  Documentation includes evidence of appropriate resident/family teaching, and the understanding of these instructions in noted in the medical record.
  • Communicate with the physician regarding residents needs, the nursing assessments, and recommendations on a daily basis, and as needed.  Coordinate ancillary services as needed.
  • Collaborate with other staff to ensure the resident’s well being.
  • Educate resident/family on appropriate health information in a timely manner and share written information/material. Perform direct patient care, utilizing established procedures, policies and standards.
  • Administer medications according to policy and procedure.
  • Administer IV’s according to policy and procedure.
  • Observe and document resident’s response to pertinent medications.
  • Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician’s orders and policy and procedure (in a timely manner).
  • Demonstrate necessary skills and knowledge to provide care for patients according to division/unit specific competencies.
  • Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner.
  • Provide personal patient care to provide comfort and well-being to the resident, acknowledging physiological and psychological needs.
  • Perform indirect patient care, utilizing established procedures, policies and standards.
  • Give a thorough, concise report at change of shift.
  • Communicate pertinent information to the appropriate person(s) in a timely manner.  Notify physician, nursing management and other chain of command regarding significant changes in resident conditions or other pertinent information.
  • Demonstrate accurate and timely documentation according to guidelines.
  • Accurately transcribe physician orders according to policy and procedure.
  • Maintain a clean, neat and safe environment for resident’s and staff according to policy and procedure.
  • Ensure that supplies and equipment necessary for resident care are stored in an organized and efficient manner.
  • Demonstrate correct and safe technique in the use of equipment according to specific product information.
  • Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.
  • Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.
  • Adhere to all facility policies and procedures regarding resident care.
  • Make daily assessment rounds and re-assess residents as needed.
  • Designate work assignments and give daily report to unit personnel.
  • Assist with answering call lights and facilitating timely response to resident needs.
  • Supervise CNA’s for completeness and quality of work and compliance with facility policy.  Address problem areas with appropriate corrective action.
  • Make rounds with physicians:  prepared to discuss labs, x-ray results, and current problems, check orders and transfer orders to MAR’s.
  • Complete daily census and other paperwork, as assigned.
  • Ensure personal property list is current.

Resident Satisfaction

  • Assist residents, family members or others with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated.  Identify yourself in a pleasant and positive manner.  Take responsibility for helping the caller.

  • Assist in the orientation and training of new staff members.

  • Investigate and follow through on unusual orders or requests for service or information.
  • Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur.
  • Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.
  • Consistently evaluate work and determine if further steps are needed to meet client expectations.
  • Take initiative to do or redo inadequate or incomplete work, even if it is not yours.
  • Ensure compliance with regulatory standards.

  • Consistently demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.

  • Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.
  • Comply with the Organization’s infection control policies and procedures including OHSA Bloodborne Pathogens Standards to ensure a safe working environment for self and others.
  • Ensure that the Organization is operated in an efficient and cost effective manner by adhering to Organization and departmental attendance policies as documented in the Employee Handbook.

Required:

Currently licensed RN.  Must be actively certified, in good standing, as a RN.  

Certified in Basic Life Support.

Preferred:

Minimum of 1year nursing experience.

Previous experience in long-term care setting

Previous charge and/or supervisory experience.

Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

Salary : $56,600 - $71,600
