What are the responsibilities and job description for the RN Case Manager - Full-time Days position at Heartland Senior Living?
- Follows the Heartland Senior Living nursing service objectives and philosophy established by the Director of Nursing Services.
- Follows standards of nursing practice. Leads by example with the staff he/she works with.
- Follows Heartland Senior Living nursing policy and procedures as well as State and Federal requirements.
- Coordinates all admissions to the facility. This includes readmissions, room moves and patient inquires. Point of contact for room moves.
- Coordinates all discharges. Discharge plans should be written and include involvement with family and community services, as appropriate.
- Assist in planning, developing, organizing, implementing, evaluating, and directing the Social Service programs of this facility.
- Schedules daily rounds to see and assess residents at their bedside and in the dining room.
- Administer medications, complete treatments as ordered, and accurately observe and monitor the residents condition on an ongoing basis.
- Maintain safety to residents and employees.
- Coordinate new employee’s site orientation with the assistance of support staff.
- Ensures that all nursing services provided to the residents under his/her case management receive adequate and timely care, which is pertinent to the resident’s health status/condition.
- Will be responsible to meet the resident/family needs by doing resident assessment, planning, evaluation and supervision of cares.
- Assures the accuracy of resident records.
- Assures that a comprehensive plan of care is established and implemented for each resident and
- that the plan is reviewed at least quarterly and within seven days of the revision of the comprehensive resident assessment.
- Follows the RAI process and consistently uses the MDS user’s manual for guidance on interpretive guidelines. Seeks out help when warnings come up or needing another opinion to correctly complete MDS. Careful to follow MDS and RCC schedules.
- Interacts with physicians to plan care of residents.
- Contributes to identify quality of care, resident rights, quality of life, or facility practice problems.
- Assures that nursing personnel caring for residents under her/his case management are following the resident’s care plan. Does teaching of staff of correct procedures.
- Provide supervision to employees in the nursing department including training and education, planning, assigning and directing work, performance appraisals, problem resolution, etc. Must be aware of good customer service and demonstrate the use of it.
- Uses quality management skills and leadership skills to fulfill the mission of Heartland Senior Living.
- Participates in department head meetings and communicates all pertinent information regarding resident care to other departments as each resident’s health status/condition changes.
- Assures personal and staff compliance to all Residents’ Rights.
- Is responsible for development and implementation of social care plans and resident assessments.
- Interview resident/families to obtain social history.
- Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
- Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
- Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of Social Service programs.
- Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaborations with community agencies), and referrals to other community agencies when specialized assistance is required.
- Evaluate social and family information and assist in determining plans for social treatment.
- Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members.
- Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
- Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed.
- Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
- Maintain a reference library of written material, laws, standards of practice, etc., necessary for complying with current standards and regulations and that will provide assistance in maintaining quality social service.
- Ensure prior authorizations through Senior Linkage Line is completed and obtained with all admissions.
- Involve the resident/family in planning objectives and goals for the resident.
- Actively participates in care plan meetings and may be responsible for involving residents and family members. Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
- Encourage the resident/family to participate in the development and review of his/her plan of care.
- Ensure that social services components of the MDS are completed accurately and signed in a timely manner.
- Review complaints and grievances made by the resident and make a written report indicating what action(s) were taken to resolve the complaint or grievance. Follow facility’s established procedures. Maintain a written record of the resident’s complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint. SSD acts as the resident advocate when necessary.
- Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.
- Participate in resident/group council meetings and family council as requested and provide support services to such councils.
- Actively participate in all vulnerable adult investigations and filings.
- Represents Heartland Senior Living to other organizations as an organization that is committed to deliver a community of services grounded in faith. Be especially conscious and careful of this when serving on community committees, boards, and in general friendships.
- Other duties as assigned.
