Looking for an experienced QDDP/Program Coordinator. Must have experience with the ID/DD Population (intellectually disabled/developmentally disabled), perform intake for individuals coming into our Supportive In-Home Program, must also develop outcomes from individual's ISP and into WAMS, Submit authorizations in WAMS, staff training, review staff’s support notes, review files for QA, point of contact for assigned staff, knowledge of CHRIS, Risk Management and WAMS, use Marketing strategies to hire staff and bring individuals into the agency.
Will be able to work remotely two days out of the week.
GENERAL STATEMENT OF RESPONSIBILITIES:
The QDDP/Program Coordinator position shall maintain the confidentiality and security of records; perform intake for individuals coming into our Supportive In-Home Program. Have knowledge of DBHDS Office of Licensing, the Office of Human Rights and Medicaid Waiver Regulations. This position additionally shall have other responsibilities and duties in areas of program development and evaluation of services being provided; personnel management, evaluation and supervisory skills; public relations, oral and written communication skills; practical experience in behavior intervention and individual programming for various levels of individuals who have developmental disabilities; This position will also have experience and knowledge with completing individual assessment tools, admissions and discharges and working with individuals with various support needs. This position will serve as the QDDP/Program Coordinator to supervise Heavenly Hands, Inc. Direct Support Professionals and be in rotation for on-call.
The Program Coordinator Shall:
Oversee and monitor program development and operations
Ensure effective and quality services
Approve hiring and terminations of personnel
Maintain knowledge of current services by attending workshops / seminars / conferences
Complete assessments and develop PCP’S
Provide supervision of all employees and contractors
Ensure proper admissions and discharge of individuals
Direct and monitor program development, implementation, and operations
Direct internal and external program audits
Ensure adherence to State licensure, human rights, and Department of Labor regulations and standards
Ensure effective, quality services and appropriate staffing
Ensure that current, accurate, appropriate individual records and other relevant documentation involving individuals and their supports are maintained.
Provide training opportunities in programming and behavioral support for individuals including behavioral assessment
Ensure training for direct care staff from the Orientation Manual for Direct Support Professionals according to licensure regulations and guidelines
Direct and oversee assessment tools to determine each individual’s support needs
Ensure and monitor the timeliness and completion of individual quarterly, and annual PCP reviews in coordination with the Support Coordinator’s Individual Person Centered Plan
Ensure confidentiality and security of individual service records
Ensure confidentiality and security of personnel records
MAJOR DUTIES:
Ensure that orientation is provided for assigned individuals and their families, legal guardian or authorized representative
Ensure that orientation and training is provided for assigned employees, contractors and that continued staff training is provided and maintained in accordance with DBHDS Licensure and Human Rights Standards and Regulations
Ensure adherence to First Aid and CPR training and other required certifications for assigned staff
Conduct performance evaluations for Direct Support Professional Staff
Ensure that health and safety management standards are implemented and followed
QUALIFICATIONS:
Bachelors degree in the field of human services or related field and 1 year of work experience with individuals who have a developmental disability diagnosis and at least one year of supervisory experience.
QDDP will have 1 year documented experience providing direct care services to individuals which includes developing, conducting, and approving assessments, PCP’s or treatment plans with individuals who have developmental disabilities (DD).

Coordinating program operations with outside agencies and organizations
Maintaining professional ethical standards
Satisfactorily completing all required training in First Aid, CPR, medication management, behavioral intervention, etc.
Communicating effectively both orally and in writing, with staff, individuals, families, and other professional and paraprofessionals
