GENERAL STATEMENT OF RESPONSIBILITIES:

  • This position shall have the primary responsibility of writing and completing the Part V of the Individual Person-Centered Plans, evaluating the Direct Support Professionals (DSPs) while they are providing services to individuals, managing the staff’s clock-in/out time in eRSP, ensuring staff has completed their support notes in a timely manner as well as maintaining confidentiality and security of records. The QDDP will have knowledge of DBHDS Office of Licensing, the Office of Human Rights and Medicaid Waiver Regulations. This position additionally shall have other responsibilities and duties in areas of public relations, oral and written communication skills; practical experience in behavior intervention and individual programming for various levels of individuals who have developmental disabilities. Ensure that current, accurate, appropriate individual records and other relevant documentation involving individuals and their supports are maintained. Complete assessments; Ensure proper admissions and discharge of individuals; Assist in the use of assessment tools to determine each individual’s support needs; Ensure and monitor the timeliness and completion of individual quarterly, and annual PCP reviews in coordination with the Support Coordinator’s Individual Person-Centered Plan; Ensure confidentiality and security of individual service records; Ensure that orientation is provided for all new individuals and their families, legal guardian or authorized representative.
  • Other duties as assigned.
  • This is an exempt position and may require some evening and weekend hours.

QUALIFICATIONS:

Baccalaureate degree in the field of human services or related field and / or 1 year of

work experience with individuals who have a developmental disability diagnosis.

PREFERRED:

Experience developing Individual Person-Centered Plans with focus on Part V.

1-year documented experience providing direct care services to individuals which includes developing, conducting, and approving assessments, PCP’s or treatment plans with individuals who have developmental disabilities (DD)

REQUIRED:

On Call Rotation

Satisfactorily completing all required training in First Aid, CPR, behavioral

intervention, etc.

Excellent written, oral, and interpersonal communication skills.

KNOWLEDGE OF:

Individual Person-Centered Plan

Support Intensity Scale (SIS) approved assessment tool

DBHDS and Human Rights Licensure Policy and Procedures

Individuals with developmental and intellectual disabilities

This position requires training and maintaining all required certifications in the following areas:

This position will report to the QDDP Manager

All Heavenly Hands, Inc employees and / or contractors are required to receive an agency overview, orientation and have a national and central registry (children) background check done within 15 business days of employment.

Job Type: Full-time

Pay: $45,000 - $51,000 annually

  • Monday thru Friday, 8a-5p. This is an exempt position and may require some evening and weekend hours.

Supplemental pay types:

  • Bonus pay

Pay: $45,000.00 - $51,000.00 per year
