APRN - Per Diem

POSITION SUMMARY:
The APRN provides medical care to Behavioral Health patients. The APRN works as a full-fledged clinician in the Department of Medicine. Protocol supervision by a staff physician is provided as required.

RESPONSIBILITIES:
Standard of Performances
  • The APRN performs as a hospitalist in the Behavioral Health Units during their shift. Is responsible for all admissions during their shift and participates in the discharge process in conjunction with psychiatry, social work services, and nursing.
  • The APRN provides quality medical care to the psychiatric patients on the units while working in tandem with psychiatry and nursing. The APRN provides acute visits (medical and behavioral) including fall assessment and hospital emergent send out.
  • The APRN helps to provide and maintain quality medical care for the patients of Hebrew Senior Care. He/she assists in the conduction, supervision, and coordination of medical care programs for HSC.
  • The APRN helps to ensure the adequacy and appropriateness of medical services and related activities (medical records documentation, patient care planning, and programming) in compliance with all applicable regulations.
  • Uses proper safety techniques and complies with all facility safety procedures.
  • Assumes other duties which may from time to time be deemed appropriate by the Chief Medical Officer.
  • Assumes leadership role.
  • Demonstrates accountability for personal, professional, ethical, and legal standards.
  • Initiates supports and facilitates change within the professional milieu in concert with institutional goals and objectives and with other members of the Department of Medicine.

Leadership and Management
  • Demonstrates leadership abilities and assumes leadership roles.
  • Consistently utilizes communication in an effective manner for goal attainment.
  • Supervises teaches and guides all department personnel when appropriate.
  • Acts as clinical/system resource to the departmental staff 24 hours/day as needed.
  • Participates in rotations of on-call duties for the organization
  • Participates in the supervision, evaluation, and development activities of members of the nursing administrative group.
  • Participates as requested in organization-wide leadership activities.
  • Assigns appropriate responsibility/authority to subordinates.
  • Responsibly delegates work. Follows through to see that assignments are accomplished and desired results are obtained.
  • Demonstrates an ability to take initiative and follow through articulately and convincingly and promotes ideas.
  • Encourages participation. Utilizes a reasonable, equitable, even-handed, and collegial approach in delegating responsibility. Recognizes contributions by others, both informally and formally.
  • Demonstrates an ability to perform duties in an organized, efficient manner, setting appropriate priorities.
  • Assists in the development of and compliance with the departmental budgetary processes.

Documentation
  • Ensure complete, consistent, and current documentation in the medical records.
  • Responsible for proper documentation of H&P, acute medical visits (including restraint/seclusion), and emergent discharges to ER.
  • Documentation includes orders, visit notes on EMR templates, and W10 completion.

Communication Skills
  • Utilizes effective communication in interpersonal relationships with patients, families, employees, and other members of the health care team.
  • Superior ability to communicate effectively in both oral and written formats.

Staff Meetings
  • The APRN helps with quality assurance programs as needed.
  • The APRN serves on appropriate Hebrew Senior Care committees when assigned.

Education
  • Masters in Nursing, Public Health or Community Health; Graduate of Nurse Practitioner program
  • Certification as a Nurse Practitioner
  • The APRN participates in all scholarly activities.

WORKING CONDITIONS
  • Coverage of 38-bed behavioral health units.
  • Working alone with MD backup during shift
  • Collaborative Agreement (if not APRN with independent licensure)
  • Workspace provided
  • On-call room provided for overnight shifts..
PERSONNEL SPECIFICATIONS
  • Registered Nurse, current Connecticut license required
  • A current Connecticut Advanced Practice Registered Nurse license is required for prescriptive privileges.
  • Superior ability to communicate effectively in both oral and written formats.
  • Superior interpersonal skills and clinical skills.
