Assisted Living Facility in Dutchess County, NY seeking an experienced Care Manager with a solid background with Adult Long Term Care, Admissions and Discharges. Experience with geriatric population. The position entails identifying, coordinating, and documenting efforts to address the needs of the residents, and the ability to work with outside agencies, hospitals, nursing homes and the families of the residents. Applicants must have a minimum of a bachelor's degree in a social services field. Must have excellent verbal and written communication skills. The ideal candidate must be able to actively interact with our residents, be proactive, be energetic and solution driven to promote resident retention.
