Specific Responsibilities:
- Create service plans for our clients following client assessments and admissions
- Ensure quality care is delivered to our clients by creating, implementing, monitoring and enforcing quality assurance policies and procedures
- Make necessary revisions to the plan of care as patient status and needs change
- Maintain open lines of communication between nursing shifts, schedulers, supervisors, aides, families and case managers
- Record all necessary information during each visit - client conditions, changes and status
- Ensure that arrangements for equipment and other necessary items and services are available
- Maintain client confidentiality
- Other job duties as needed
Qualifications:
- Must be 18 years of age or older
- Valid driver's license and reliable transportation
- Must be eligible to work in the U.S with an Ohio state ID
- Must have a current RN or LPN license in the state of Ohio
- Must be CPR/First aid certified
