Admissions and Social Services Assistant

Hendersonville Health and Rehab is seeking an Admissions and Social Services Assistant to join our team of dedicated health care professionals.

Responsibilities:

• Assess nature of patients' situations by interviewing and reviewing personal history

• Develop and execute individual treatment plans/care plans

• Communicate patient updates to all relevant parties

• Facilitate referrals to other healthcare professionals and programs as needed

• Proactively reach out to and responds to inquiries from hospital discharge planners, families, and other referral sources

• Serve as the initial liaison with residents and families on facility services

• Speak with residents or residents’ representative to obtain necessary personal and financial data to determine eligibility for admission

• Assign accommodations for new admissions related to nursing care needs, resident and residents’ family’s preference, availability of space, and other information.

• Ensure all admissions paperwork is completed thoroughly, accurately and on a timely basis.

• Maintain records of admissions and discharges and compile occupancy census data. Participate in meetings to alert appropriate staff members of projected admissions, room changes, and discharges

• Maintain knowledge of Medicare & Medicaid regulations to ensure services are in compliance with professional boarding standards, state and federal regulatory requirements.

Qualifications:

• Skilled Nursing Facility/Long Term Care experience is preferred

• Possess the ability to deal tactfully with personnel, residents, visitors, and the general public

• The ideal candidate should have a passion for working with seniors, a high degree of customer service and extensive marketing and sales experience

• Compassionate and caring demeanor

• Excellent written and verbal communication skills
