JOB SUMMARY |
- The Hospice Patient Placement Specialist receives phone calls on hospice referral line and directs calls to appropriate discipline. Performs data entry of patient information to begin the admission process for hospice care. Verifies patient insurance benefits. Scans medical records into patient EMR. Keeps patient supplies/linens ordered and stocked. Functions under the direction, instruction, and supervision of the Admissions Coordinator/Hospice Team Leader or Nurse Manager.
