Care Coordinator - Behavioral Health Urgent Care
Do you want to help others and make a difference? Heritage has created a team to provide support and guidance to individuals seeking a wide-range of needs from our Behavioral Health Urgent Care program. The team is seeking to add the following members to our Care Coordination team:
- Counselors
- Engagement Specialists
- Care Coordination
- Crisis response team members
Responsibilities include:
- Provide a supportive and helping response to individuals presenting at Heritage needing support, assistance, or referral and linkage
- Provide assistance in assisting individuals in identifying, applying, and accessing community resources
- Referral and linkage to a wide array of community and agency resources including mental health, substance use, healthcare, budgeting, housing, education, employment, and social support
- Assist individuals in creating a wellness toolbox, developing a daily plan, identifying stressors and early warning sign, creating an action plan when stressors increase, and developing a crisis plan
- Participate in a team-based approach to providing treatment in collaboration with clinical teams, healthcare providers, community organizations, and other partners invested in an individual's recovery
- Short-term case management or counseling
Knowledge, Skills, and Abilities:
- Ability to work with individuals with a diagnosis of mental health and/or substance use in a kind, caring and supportive approach that instills hope
- Ability to rapport quickly with individual seeking help
- Ability to be flexible to help anyone, anywhere, and anytime
Education and Experience:
- Experience working with individuals with a mental health or substance use diagnosis is highly desired
- Individuals with a Bachelor's Degree or Master's Degree in psychology, social work, or similar human services field for counselor roles
- Individuals with lived experience of a mental health or substance use diagnosis is desired for Engagement Specialist role
