Social Worker | Full-Time Position

A successful Social Worker has the following:

- Possess assessment and evaluation skills to navigate residents and family situations
- Expertise in conflict mediation, abuse & neglect, advance directives, crisis management, and government programs
- Exhibit ability to make responsible choice and decisions that are in residents’ best interest
- Experience in case management, interventional services, mental heath, behavior analysis, treatment planning and/or social services is preferred
Responsibilities:
- Participates in multi-disciplinary approach to reduce psychosocial barriers to health outcomes and to support resident engagement, understanding, and adaptation to chronic disease or relocation stress
- Document detailed notes in the residents’ electronic medical records that reflect the therapeutic plan and specific aspects of the ISP
- Maintain comfort, privacy, and dignity of our residents
- Report and investigate reportable or sentinel events for their root cause, including reviewing progress notes and incident reports and developing a plan to minimize the reoccurrence of the event
- Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance
Requirements:
- Ability to speak, read and write English
- Valid and good standing State of Wisconsin Social Work Licensure (one of the following): Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), and/or Licensed Clinical Social Worker (LCSW)
- 2 years pf experience as a licensed social worker
- Flexible and able to handle multiple variations on daily schedule
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines

- Receive continuous training from mentor as you continue to provide exceptional care to our residents
