Memory Care Coordinator

Responsible for the day-to-day management and success of the Memory Care Community, including planning, organizing and coordinating special needs programs. This includes providing a therapeutic environment that maintains our residents the highest level of physical, social and psychological well-being. The role requires educating the community team to engage in life enrichment activities. You will also provide team member supervision and training, and act as a liaison and advocate for our residents and their families. This is a hands-on position requiring occasional participation in assisting residents with ADLs.

Required Qualifications

  • At least three (3) years working intimately with Alzheimer’s or dementia patients and a minimum 5 years’ experience working with the geriatric population
  • A minimum of two (2 ) years of supervisory experience in an assisted living, skilled nursing, clinical or similar healthcare related field is preferred
  • Experience planning and administering in-service education programs
  • Demonstrate exceptional leadership skills as a mentor who can lead by example
  • Possess a working knowledge of medical and nursing terminology
  • Knowledge of geriatric care needs, supervision and administration
  • Excellent communication skills, both verbal and written, with the ability to speak clearly, concisely and confidently with residents and their families, physicians, property personnel and other key stakeholders
  • Exceptional multitasking, organizational and time management skills a must
  • Ability and flexibility to excel despite frequent work interruptions
  • Reset priorities in order to complete work responsibilities in a timely manner
  • Apply discretion in handling confidential information
  • Ability to cope with the mental and emotional stress related to working with special needs population
  • Strong computer skills and proficiency with Microsoft Office

Additional responsibilities include:

  • Ensuring that resident care and management needs are sufficiently and appropriately assessed
  • Serving as the liaison between residents, the resident’s legal representatives, their families, administrative staff and Embassy Memory Care community-based service providers
  • Motivating and developing team members, including providing in-service education of the special needs program to Reflections area team members on health/wellness related topics
  • Leading the daily operations of the Empower Memory Care Community and ensuring compliance with company and Department of Health (DOH) regulatory policies and standards regarding resident admission and discharge procedure as well as compiling appropriate medical case notes and other required documentation
  • Scheduling and overseeing team members and assisting with medication management
  • Create and design monthly life enrichment calendar according to Embassy Healthcare standards and ensure programs are taking place as noted.
  •  In conjunction with the dietary team, ensure that the Embassy Elevated Dining Standards are in place including managerial assistance for a minimum of 2 meals per workday.
  • Plans, Develops, organizes, implements and directs groups inside and outside the community ensuring the program meets the mental, physical, spiritual and social needs of the resident.
  • Work with Sales and Marketing Director and Resident Services Director to review all prospective move-in to the Memory Care Program.

·       Work with Sales and Marketing Director on community outreach plan to ensure strong partnerships with industry partners for the purpose of identifying and partnering with local service resources.

·       Ensure proper documentation of all Memory Care Program services. This includes quarterly progress notes on all Memory Care Residents regarding life enrichment activities and social needs.

·       Work with families when a transfer/move-out from the Memory Care Program is necessary to ensure a smooth transaction.

·       Treats all residents with dignity and respect.  Promotes and protects all residents’ rights.

·       Establishes a culture of compliance by adhering to all facility policies and procedures.  Complies with standards of business conduct, and state/federal regulations and guidelines.

·       Follows appropriate safety and hygiene measures at all times to protect residents and themselves.

·       Maintains confidentiality of protected health information, including verbal, written, and electronic communications.

·       Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.  Reports any retaliation or discrimination to HR or compliance officer.

·       Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor/Administrator/Abuse Director.  Protects residents from abuse, and cooperates with all investigations. 

·       Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. 

·       Participates in all life safety and emergency drills and trainings.  Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan.

·       Reports work-related injuries and illnesses immediately to supervisor.

·       Follows established infection control policies and procedures.

·       As a condition of employment, completes all assigned training and skills competency.

·       Meets general health requirements according to facility policy, including medical and physical exams and checking Memory Care Community status to various infectious diseases.

  • Ability to work beyond normal working hours and on weekends and holidays when necessary.
  • Ability to assist in evacuation of residents during emergency situations.
  • Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
  • May be subject to falls, burns from equipment, and/or odors throughout the day; encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants.
  • Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis.
  • May be subject to hostile or emotional residents, family members, visitors or personnel.
