What are the responsibilities and job description for the MassSTART Case Manager position at HFLW Brand?

The MassSTART Case Worker will provide therapeutic case management services in the school systems and homes of children and families identified as needing support using a substance abuse prevention lens of care based in Boston, MA.

How You'll Be Making a Difference

  • Collaborate with school personnel to generate program referrals and support the collaboration between school and families/caregivers.
  • Participate in school and/or service-based meetings related to enrolled students and families/caregivers.
  • Participate in the planning and execution of program activities and events facilitated for students and families/caregivers, including student groups, family activities, and field trips.
  • Capacity to deliver services in the language most appropriate for the family
  • Conduct individual student check-ins based on identified student needs; facilitate weekly social and life skills groups to enrolled students
  • Develop and maintain collaborative, long-term relationships with families/caregivers, which include regular communication and home visits.  Provide support to families to address a variety of issues which may include, family communication, parenting skills, substance abuse, domestic violence, self abusive behavior and past trauma
  • Assist students and families/caregivers with accessing resources and strengthening their support systems through referrals, advocacy, and collaboration.
  • Administer written assessments, summary reports, documentation, treatment planning, and safety planning to appropriate parties in a timely, well written manner.  Submit reports, documentation and billing in a timely manner. Ensure that documentation meets utilization review and quality assurance standards
  • Continuously communicate with client collaterals and providers of supportive services.  This may include extensive telephone contact and scheduling and attending case conference meetings. Facilitate communication between various providers.
  • Plan, assess and provide all treatment interventions in an active partnership with collaterals.   Work as part of interdisciplinary team to provide quality services.
  • Attend staff meetings.  Present client material, consult with supervisor and team members on challenging and difficult client issues. Participate in regular individual and group supervision.  Attend clinical training and safety practice training.

Qualifications

  • Bachelors Degree
  • Bilingual - Spanish
  • Experience with behavior management systems, treatment planning, crisis intervention, case management and Boston Area resources and services  
  • Excellent written and verbal communication skills 
  • Knowledge of SAMHSA and other local child and family advocate agencies is preferred
  • Computer literate including Microsoft Office, Word, Excel and Internet Explorer. Additional ability to learn new software applications when necessary
  • Must be able to work in a flexible work-location arrangement using agency-issued laptops (or equivalent mobile computer).
  • Experience with providing services to children and families in an urban setting; fluency in Spanish highly preferred

Driving Requirements

  • May be required to operate a vehicle 
  • May be required to have a valid driver’s license and a “satisfactory” driving record
  • May be required to have access to a personal vehicle for use during work hours
  • May be required to use personal vehicle to transport clients on occasion if/when needed
  • If required to transport clients, must maintain the minimum amounts of liability insurance as required by The Home and/or other regulatory requirements and vehicle registration must be current
