The Case Manager will work with the Veteran's Response Team in both Cherokee and Cobb Counties. The Case Manager will work with a case load of veterans in need of mental health services who may be at risk to suicide.
Job Responsibilities include:
- Provide community support interventions for individuals in the program and in community, including school and home and complete clinical documentation
- Teach individual skills to improve day-to day function and overall well-being i.e. skills to manage mental health symptoms, life skills, social skills, etc.
- Offer the youth and family a single point of contact to ensure access and a coordinated response to tangible youth needs such as food, shelter, clothing, transportation, education, etc.
- Assists in completing reports and requirements per programmatic requirements
- Assists in recruitment of members, including regular communication with referral sources and reaching out to families once they are referred to the program.
- Prepare group activities and provide supervision during activities
- Facilitates life skills groups, including coping skills, communication and interpersonal skills
