Pharmacists at Hillcrest Hospital Claremore compound, fill and dispense pharmaceutical preparations in the pharmacy within the guidelines set forth by the state of Oklahoma and the Federal Government. Pharmacists work under the Oklahoma general supervision of the Director of Pharmacy and supervise and instruct pharmacy technicians. They also participate in performance improvement activities of the department.

Responsibilities
What You'll Do
The Pharmacist II (Clinical Pharmacist) will be expected to:
- Assist with the development of clinical programs, clinical monitoring, medication reconciliation, formulary management, cost containment programs, and improved pharmacoeconomic outcomes
- Assure pharmacy services and activities are carried out to meet goals and standards
- Ensure safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, protocols, and national guidelines
- Assist with the development of treatment guidelines, protocols, formulary changes, and clinical pathways as approved by appropriate committees
- Monitor drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, adverse drug reactions, appropriateness of drug and dose, and therapeutic duplications
- Coordinate the selection and safe use of alternative medications in response to medication shortages
- Facilitate the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the Pharmacy and Therapeutics Committee (P&T)
- Serve as a subject matter expert for the committee
- Prepare and present drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports, and pharmacy interventions summary to P&T
- Support the pharmacy's pharmacoeconomic efforts
- Read and evaluate the medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs
- Provide training and education to pharmacy team regarding pharmaceutical care and pharmacoeconomic initiatives
- Review the use of targeted medications for compliance with current programs
- Provide clinical consultation and clarification to practitioners. Suggest appropriate, cost effective therapeutic alternatives to medical staff as needed
- Provide accurate, adequate, and timely drug information to professional staff
- Ensure productive, collaborative, relationships with staff members and within the community
- Participate in community education programs as available
- Contribute to the advancement of the profession through presentation at local, state, and national meetings
- Participate in the quality improvement activities of the department, with a strong focus on clinical activities
- Prepare reports on pharmaceutical care and pharmacoeconomic activities including financial impact
- Assist in development and maintenance of information systems and other technology application within the pharmacy
- Attend and participate in departmental meetings, in-services, and quality improvement programs
- Maintain all required training and education per department requirements
- Adhere to hospital and departmental policies and procedures. Comply with all requirements related to risk management, safely, security, fire, and infection control. Comply with all applicable state, local, and federal regulations
- Perform as Staff Pharmacist as needed
Qualifications
What You'll Bring
Experience
- Minimum of three years hospital clinical pharmacy experience and/or postgraduate year one pharmacy residency preferred
Education/Certification
- Current license to practice pharmacy in Oklahoma
- Graduate of college of pharmacy accredited by ACPE or equivalent - PharmD preferred
- Board of Pharmacy Specialties Certification preferred
- Basic Life Support (BLS)
- Advanced Cardiac Life Support (ACLS) preferred
- Active membership in American Society of Health-System Pharmacists
- Training and proficiency in age specific therapies for hospital patients
- Neonatal, pediatric, adolescent, adult and/or geriatric
