POSITION OVERVIEW:
This position is responsible for the coordination of high-quality services for the residents living in the memory care wings at The Commons. Provide care management oversight and personal care assistance which will include the provision of protective oversight, medication administration, hospitality, and customer services to the residents and their families.
This position has a full-time schedule of Tuesday-Saturday shifts.
This position has a starting wage range of $24 - $25.20 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Create, implement, revise, and continuously update policies and procedures to maintain efficiency, professionalism, quality assurance, safety, and regulatory compliance.
2. Hiring and managing assigned care specialists; responsibilities include leading, coaching, mentoring, training, behavior modeling, corrective actions, etc.
3. Conduct initial assessments of new residents.
4. Write effective and timely individualized care plans, and update them as needed.
5. Provide regular effective communication with resident's families and conduct care conferences as necessary.
6. Resolving staff and/or resident issues and concerns.
7. Coordination of move-ins, including collaboration with the business office and sales/marketing department.
8. Reassessments in compliance with the State regulations.
9. Providing protective oversight to all residents; ensuring staff responsibilities are completed.
10. Planning of weekly activity programs and calendars; collaborating with the Life Enrichment Director to acquire necessary resources.
11. Ensure all staff receive high quality orientation and training that meets state regulations (QMAP, CPR/First Aid, Food Handlers, other).
12. Providing direct care as needed for residents.
13. Coordinate the dementia certification program offerings in collaboration with Senior Daybreak Director.
GENERAL DUTIES AND RESPONSIBLITIES
1. Participate on the CORE Leadership Team and attend meetings as required.
2. Participate in weekly case review and supervision.
3. Participate in monthly meeting as required.
4. Other duties as assigned.
