Social Services Coordinator

Job Overview:
We are seeking a highly organized and experienced Service Coordinator to join our team. As a Service Coordinator, you will be responsible for managing and coordinating services for individuals with developmental disabilities, Children and Seniors. Your role will involve supervising staff, program management, administrative tasks, and building relationships with clients and family members.
Responsibilities:
- Develop and implement service plans for individuals.
- Coordinate and monitor services provided to clients, ensuring they are receiving appropriate care and support
- Conduct assessments to determine client needs and develop individualized service plans
- Supervise a team of direct support professionals, providing guidance and support
- Collaborate with community resources to ensure access to necessary services
- Monitor program outcomes and make recommendations for program development and improvement
- Maintain accurate documentation and records of client progress and service delivery
- Provide crisis intervention as needed
- Build positive relationships with clients, families, caregivers, and health plans
Experience:
- Experience in the Social Services Field preferred
- Strong knowledge of program management principles and practices
- Experience in supervising staff members
- Excellent organizational skills with the ability to manage multiple tasks simultaneously
- Strong communication skills, both written and verbal
- Ability to build rapport and maintain effective relationships with clients, families, caregivers, and community partners
Bilingual Spanish/English, Spanish/ Creole
Job Type: Full-time
Pay: $15.00 - $16.00 per hour
Expected hours: 40 per week
Schedule:
- Day shift
- Monday to Friday
