Job Description

 The Immunization Action Plan Coordinator serves as part of the Health Protection and Education Center. Under the supervision of the Center Director, the coordinator manages the Hartford and Greater Hartford Immunization Action Plan (IAP) response initiatives ensuring program efficacy, deliverables, and implementation of methods of increased performance measures.

 

Connecticut’s IAP helps raise vaccination rates for children in our state. It operates in areas with the highest risk of low immunization rates and focuses on preschool children who may not have received the vaccinations required to start school. The IAP Coordinator works closely with local health departments, public and private immunization providers, hospitals, WIC offices, Head Start programs, schools and pre-schools, and other maternal and child health organizations that serve young children. By collaborating with these groups, the local IAP can assess the community's needs, make and receive referrals for at-risk children, and share the latest immunization advice with parents and guardians.

IAP program goals are:

  • Contact and engage the parents and guardians of under-immunized children.
  • Educate people about the importance of immunization.
  • Identify ways to improve vaccine delivery.
  • Increased immunization rates in the towns or communities they service.
  • Successfully link at-risk children to a “medical home” so they receive vaccinations, and their immunization is tracked.

 The hours of work are 35 per week with the required to travel to site locations.

 This is a grant-funded position. The position is contingent on grant funding.

 

Knowledge, Skills, and Abilities

  • Knowledge of child and adult immunization /vaccine schedules and ACIP recommendations.
  • Knowledge of storage and handling of vaccines
  • Knowledge of infant Hepatitis B requirements
  • Experience with community education in regard of vaccination requirements
  • Ability to work with primary care clinics, hospitals, and community agencies.
  • Ability to work independently.
  • Ability to follow budgets and work collaboratively with staff members and external partners.
  • Ability to think creatively and develop innovative solutions to complex problems.
  • Ability to display professionalism through marketing and outreach efforts.
  • Ability to work flexible work hours including pre-scheduled nights and weekends.
  • Strong communication and organizational skills

 

Qualifications

Degree in Public Health, Nursing Education, Health Sciences, Health Promotion, or any related field and three (3) years of experience in a field related to public health, health promotion, healthcare or health sciences providing community education, outreach, training and employment, direct care services or closely related field to a diverse population.

Must have and maintain a valid Driver’s License

 Preferred Qualifications:

  • Experience with infectious disease prevention and control, clinic management, immunization/ vaccine administration, vaccine storage and handling, and/or community education.
  • Experience with supervision of staff.
  • Bilingual – Spanish
