Holly Hill Child and Family Solutions is seeking a Residential Case Manager to join our growing team. The Residential Case Manager position is responsible to the Senior Director. The Residential Case Manager will be responsible for providing client admissions meetings, identifying needed records, obtaining required documents, completing reports and managing client records and files. The Residential Case Manager is responsible for coordinating with external service providers and DCBS to ensure client needs are met. Additional duties include ensuring that program is in compliance with regulations required for treatment, medical services and medication for children in the program.

Responsibilities:

• Obtain all medical records for incoming and discharging clients including release of medication, release of care care, discharge summaries, medical passports, etc.
• Assist in school enrollment of new clients and attend school meeting to ensure education needs are met.
• Provide transportation when necessary to medical appointments.
• Provide necessary documentation to medical care providers for treatment and payment. Manage any billing issues with providers.
• Coordinate and attend appointments with psychiatrist to communicate medication needs and ensure appropriate follow up.
• Maintain a regular schedule of counting and monitoring medication administration; reviewing reports of medication documentation errors.
• As a member of the treatment team, assures that each child’s treatment, care and health and safety needs are met with the highest quality of services possible (ensuring educational, medical/dental and mental health needs are being met)
• Actively plan for discharge throughout treatment and coordinate appropriate aftercare services.
• Position runs M-F (10:00am to 6:00pm) with flexibility on Tuesday mornings for team meetings.
• Provide clinical on-call coverage on a rotating basis (Masters in counseling or social work will be on clinical on call/ Bachelor's will be supervisory on call).

Requirements:

• Bachelor’s degree in social work, human services or a related field.
• Training experience, experience in a health care or mental health setting working with adolescents and/or children. Knowledge of psychotropic medications and basic health care practices.
• Good written and oral communication skills. Must have time management skills and ability to be flexible. Possess skills of assertiveness and ability to present self in a professional manner. Knowledge of basic health care, medications and some knowledge of mental health care and psychotropic medications. Must have a valid driver’s license be able to transport clients.
