Requirements:
Education: Master’s required, Doctorate preferred. Knowledge of home exercise programs and equipment, Medicare, Medicaid and third party insurance reimbursement, State, Federal and JCAHO standards for home care. At least 5 years of clinical experience; and at least 3 Years in the Homecare CHHA Setting. Must have a valid driver’s license and access to an automobile. Job requires extensive standing, walking, lifting crouching, stooping, reaching, kneeling, stair climbing, writing, driving and phone use. Responsibilities include making home visits in various areas during inclement weather and exposure to variable environmental conditions in the home setting care experience preferred. Licensed to Practice Physical Therapy or Occupational Therapy in the State of New Jersey.
Essential Functions of the Job:
- Effectively Supervise Rehabilitation Staff. Monitors Staff Productivity and Ensures that criteria based performance evaluations are completed in a timely basis according to policy.
- Effectively recruit and interview candidates for employment and ensures staff qualifications are in compliance with State and Federal Requirements.
- Perform HR duties including Managing Staff Requirements for Continued Employment such as License Renewal and Employee Health Screenings. Maintain Rehab Staff
- Assess the Rehab Staff’s Educational needs and Develop Educational Programs. Hold Regular Rehab Meetings.
- Work in collaboration with Education Department to ensure that Staff is updated and compliant with Regulatory and practice Updates.
- Monitor and Manage Rehab Referrals and Assignments to ensure Timely Provision of Care to Patients
- Approves Payroll and ensures that billing Statements are accurate and matches Visits made. Ensures Timely Payroll Submission and effectively Monitor Time and Attendance.
- Oversee clinical compliance practice of Rehab staff to ensure appropriate utilization of services.
- Work with Quality Improvement Team to develop Performance Improvement Programs and Policies
- Communicate with managed care organizations to facilitate authorization of services, documents communication, and coordinate services
- Participate in Regulatory Compliance Surveys and initiate Rehab Corrective Action Plans and monitor Performance as needed to ensure compliance with Regulatory Mandates.
- Collaborate with Other Interdisciplinary Team to ensure a Comprehensive Care approach in Patient Care.
- Delegates Tasks to therapists and support staff and obtains follow up regarding tasks assigned.
- Report to Administrative Team all pertinent Rehabilitation Department Activities and Concerns
- Ensures Staff Competency by performing Field Competency Visits, chart audits and identifying areas that needs education and training focus. Will perform joint visits for consultation on Complicated Cases and eligibility for Maintenance Programs.
- Participate in Committees both Internal and External to help establish Best Practice Standards and be updated on Industry Operations.
- Perform Clinical Visits as needed.
- Performs Start of Care/OASIS visits, Initial Evaluations and follow up visits as needed.
- Performs training of all new staff including OASIS training, EMR training and field visit training.
- Demonstrates compliance with the Hospital and Home Care dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Understands and participates in the Home Care Performance Improvement Plan and process.
- Observes Hospital policy concerning Fire and Safety regulations, Infection Control Standards, and other rules governing conduct while at work.
- Demonstrates a commitment to own work schedule by adhering to attendance standards and obtaining coverage for cases during illness or vacations.
- Provides care and services to reflect a commitment to the HNH Mission Statement.
- Effectively oversees the utilization of Physical, Occupational, and Speech Therapy Services for Home Care.
- Monitors equipment for use by patients
- Monitors department issues and seeks out resolution
- Attends to daily staff needs
- Oversees students/volunteers assigned to area
- Represents hospital/department in community/department events.
- Performs Performance Improvement Activities as per the Department / Hospital Performance Improvement Plan
- Oversees peer review on documentation and observation of staff.
- Ensures that infection control procedures are followed.
