SUMMARY OF JOB:
Assumes responsibility for coordinating and facilitating quality improvement activities, serving as best practice expert for therapy discipline.

RECRUITMENT REQUIREMENTS:
· MS PT, OT or Speech Therapy program
· Minimum of five years experience in a healthcare setting, e.g., hospital, home health care, long-term care setting required.
· 3 year home care/hospice experience required
· 1 year of quality improvement experience in a healthcare setting preferred
· Knowledgeable of federal and state home care/hospice regulations.
· Knowledgeable of home care/hospice accreditation and compliance regulations and activities.
· Knowledgeable of quality improvement principles and activities.
· Proficient in Microsoft office and other PC applications.
· Experience in electronic medical record documentation preferred.
· Ability to perform Root Cause Analysis
· Good written and oral communication skills.
· Ability to travel to other locations as necessary.
LICENSE AND REGULATORY REQUIREMENTS:
· PT, OT or Speech license required.
· Valid driver's license.
