RVCC is looking to hire Community Health Worker for our School-Based Health Centers!

This is a 10-month school-year position at one of our School-Based Health Centers. This program provides comprehensive health care on-site at public schools in Holyoke. The CHW will attend community events, work as a team, and provide outreach, assessments, and goal-setting with clients and their families.
Job Responsibilities:
To support all aspects of RVCC's Mission statement & clientele
To provide culturally appropriate health education, information, and outreach in community-based settings
To establish trusting relationships with patients and their families while providing general support & encouragement
To perform initial and periodic needs assessments, including assessing barriers (i.e. transportation, community barriers, social supports); patient and family or caregiver preferences and language, literacy and cultural preferences
To follow up with patients via phone calls, home visits, and visits to other community agencies
Attend community meetings and health fairs to understand community issues and provide information to community assists families, groups and communities in accessing health related services
Identify available community-based resources and actively manage appropriate referrals, access, engagement, follow-up and coordination of services
Use health information technology to link to services and resources and communicate among team members, providers, community partners and patients and their families/caregivers
Attend relevant trainings
Collect and report on data for program evaluation
Job Requirements:
Education Level - High School diploma or equivalent required. Successful completion of a CHW training program preferred
Years of Experience - 1 to 2 years of experience working in a community-based setting
Skills/Abilities - Bilingual (English/Spanish) required. Excellent oral and written communication skills; ability to establish positive, supportive and trusting relationships with patients and colleagues and to work collaboratively and effectively within a team; knowledge of community social services and resources; knowledge of medical terminology and basic computer skills required. Extensive understanding of the Holyoke community including community resources.
Licensing - CHW certification preferred
Salary Range:
