Job Overview: We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality care to patients in the comfort of their own homes. This is a rewarding opportunity to make a positive impact on the lives of individuals in need. Responsibilities: - Conduct diagnostic evaluations and assessments to determine patient needs - Develop and implement personalized care plans for each patient - Provide primary care services, including administering medications, monitoring vital signs, and managing pain - Perform wound care and other specialized treatments as needed - Collaborate with physicians and other healthcare professionals to coordinate patient care - Educate patients and their families on managing their health conditions and promoting wellness - Document patient progress, treatment plans, and outcomes accurately and timely - Ensure compliance with all regulatory guidelines and standards Experience: - Minimum of 2 years of experience as a Registered Nurse in acute care or home health setting - Strong clinical skills with the ability to provide comprehensive patient care - Knowledge of managed care principles and practices - Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members - Proficient in physiology knowledge and understanding of various medical conditions. This position also requires being on an on call rotation

Job Type: Full-time

Pay: $30.00 - $32.00 per hour

Expected hours: 40 per week
