What are the responsibilities and job description for the Office LPN position at Home Health Care 2000?
The Office LPN/Position is responsible for charting clinical staff/patient encounters in real time, such as during a home health care visit in patient's home or from a clinician's dictated information pertaining to the patient's encounter.
- Responds appropriately to incoming office calls relating to the clinical needs of our organization
- Works with additional office staff in providing clear communication and documentation of EMR and other office protocol
- Process timely authorization of services for patient billing
- Follows the plan of care in providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
- Provides clear communication and accurate and timely documentation of EMR consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.•
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the clinician and/or clinical supervisor.
Requirements•
- Graduate of an accredited practical nurse or vocational nursing program.
- Has three years nursing experience
- Community/home health or medical/surgical experience is preferred
- Must be proficient in typing and good at spelling, punctuation, grammar, and oral communication
- Must be able to listen to complex Medical information and summarize in a clear, complete and concise fashion
- Must be possess excellent organizational skills
- Must be proficient in medical diagnosis coding
