Care Specialist Job Description

A Care Specialist is a liaison to clients and caregivers to provide an exceptional client experience. This position works with the RN Care Specialist to report changing client needs, train caregivers on specific client’s care needs, and works with formal and informal social support systems and families to ensure client’s needs are met. This position will also work as a Caregiver when needed.

Responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with Agency policies and procedures.

Reports to: RN Care Specialist

  • Introduce Caregivers to new clients to successfully start services
  • Work shifts to train caregivers or fill in when needed to cover open shifts.
  • Document and report observations on caregiver performance
  • Prepares complete and accurate case notes; writes correspondence and other written materials.
  • Develops and maintains working relationships with other department staff and partner agencies to coordinate services for families.
  • Interacts with other professionals on client’s behalf which may include physicians, hospice, home health personnel, and funding source.
  • Participate in on-call rotation as well as providing scheduling assistance during office hours as needed.
  • Ensure the client’s safety and security by observing clients and caregivers.
  • Creating and updating client care plans as needed to ensure Caregivers are successful in all areas of care need.
  • Complete and maintain records of daily activities, observations, and direct hours of service.
  • Collaborating with patients' families, friends and social supports
  • Make decisions and solve problems.
  • Develop and maintain constructive and cooperative working relationships with others.
  • Participate in the daily Scheduling stand up meeting.
  • Available during business hours for client needs and on-call after hours when scheduled.
  • Attending orientation, in-service training sessions and staff meetings as needed.
  • Ensure the success of each client to enable them to live independently in their home. Make recommendations for service enhancements including increased care hours, sharing information on Aloe Care and provide referrals to other healthcare providers, such as Hospice when needed to facilitate patient's treatments.
  • Maintaining records of case management activities.
  • Backup to colleagues as needed.

The ideal candidate will have outstanding organizational and time management skills, and the ability to prioritize. Do you enjoy caregiving, and want to mentor and train new caregiver. Educate new caregivers and ensure they are set up for success.

They will also be confident and outgoing and comfortable going into the clients home to train and introduce new caregivers to the client. If you have a willingness to take charge, you may be the perfect candidate for this position.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour

  • Burlington, WI: Reliably commute or planning to relocate before starting work (Required)

Experience:

  • Caregiving: 5 years (Preferred)
