Job purpose
The general purpose of your job is to keep children who are clients of HOTI safe at all times and to be respectful of the children and their families as valued customers.
The primary purpose of your job is to help children and their families through family challenges and crisis. Targeted Case Managers advocate for the needs of children and youth living at home and in the community. The Targeted Case Manager provides comprehensive, targeted case management services to severely mentally ill and severely emotionally disabled clients within their specific program. The individual will provide a wide variety of duties including, but not limited to, coordinating services among various providers and advocacy to enable clients to obtain access to needed mental health, medical, social, educational and other support services. They are instrumental in supporting the adolescent through the transition to adult services when the time comes, or work with children who need prescribed psychotropic medications.
Duties & Responsibilities
  • Responsible for conducting a comprehensive assessment and a periodic reassessment of the client’s medical, social, and functional status and identification of the client’s strengths and needs.
  • Responsible for assisting an individual to gain access to identified medical, social, educational, and other service needs.
  • Communicates with DCBS, family, school, etc. regarding progress in care and facilitates a team approach to develop individual case plans.
  • Provides guidance and support to clients according to treatment plan goals and objectives or links client with community resources related to treatment goals.
  • Communicate and collaborate with internal and external behavioral health providers to meet client needs and support client outcomes.
  • Conduct home, school or community visits based on individual child and family need.
  • Provides transportation, for service recipients, to community resources as necessary.
  • Participate in weekly team consultation meeting, and individual supervision as needed.
  • Comply with all service contract requirements.
  • Complete and submit all clinical and agency paperwork in a timely and accurate manner to meet the billing standards of all appropriate payer sources.
  • Establish and maintain positive working relationships with referring agency staff and other community professionals.
  • Represent the agency in community meetings and on interagency teams as directed.
  • Maintain training and Continuing Education Units as required by Kentucky Statutes and Regulations.
  • Maintains case load of a minimum of 18 billable cases per month.
  • Other duties as assigned.
Qualifications
  • Bachelor of Arts or Science degree in a behavioral science; is required.
  • Behavioral science means: psychology, sociology, social work, family studies, human services, counseling, nursing, behavioral analysis, public health, special education, gerontology, recreational therapy, education, occupational therapy, physical therapy, speech-language pathology, rehabilitation counseling, or faith based education.
  • One (1) year of full time employment experience, after completing the educational requirements, working directly with:
  • Adults in a human service setting; or
  • Individuals under the age of twenty-one (21) in a human service setting is required.
  • A master’s degree in a behavioral science may substitute for the one (1) year of full-time employment experience.
  • Successful completion of required TCM training, within six (6) months of employment as a TCM, is required.
  • Successful completion of required continuing education every three (3) years thereafter is required.
  • Valid driver’s license and insurable driving record is required.
  • Must possess the willingness to work harmoniously with co-workers, community partners, clients, and families.
  • Must possess the ability to plan, organize, develop, and guide the implementation of clinically appropriate interventions for the children and families we serve.
  • Demonstrated ability to work with diverse individuals, exercise discretion, maintain client confidentiality, and communicate effectively verbally and in writing is required.
  • Demonstrated ability to work independently and exercise professional decision making skills and judgment is required.
Working conditions
  • Requires working flexible hours, occasionally including evenings, weekends and holidays.
  • Requires extensive travel to client homes and throughout the community.
  • The duties of this position are performed in a mobile environment but also requires frequent travel to community partner locations and client homes.
  • While performing the duties of this position, the employee is exposed to weather conditions prevalent at the time.
Physical requirements

  • Ability to move across uneven terrain, remain sitting for long periods of time, ascend and descend stairs (with or without reliable handrail), and navigate within cluttered, disorganized or unclean home environments.
  • Ability to use a computer to access the online client information system to enter required clinical documentation.
