As a Health Coach Awareness specialist, you will work closely with our clients and their families to promote health and wellness through education and support. You will act as a trusted advisor, empowering clients to take control of their health and well-being.

Key Responsibilities:

- Develop individualized health and wellness plans for clients based on their unique needs and goals

- Conduct assessments and provide ongoing monitoring and evaluation of clients' progress

- Educate clients on preventive health measures and promote healthy lifestyle choices

- Provide guidance and support to clients in managing chronic health conditions

- Collaborate with multi-disciplinary teams to ensure a holistic approach to care

- Offer emotional support and encouragement to clients and their families

- Keep thorough and accurate records of client interactions and progress

- Stay up-to-date with current health and wellness trends and research to better serve our clients

Qualifications:

- Bachelor's degree in health promotion, health education, or related field

- Previous experience in health coaching, patient education, or related field

- Strong knowledge of health and wellness principles and practices

- Excellent communication and interpersonal skills

- Empathetic and compassionate attitude towards clients and their families

- Ability to work independently and as a part of a team

- Strong organizational and time management skills

- Proficiency in computer programs and electronic medical record systems
